The Care Quality Commission (CQC) said that 428 personnel at the acute hospital trusts contacted the watchdog to reveal major patient safety failures in their hospitals in the 12 months to September 2013.
CQC said they have checked all the complaints to ensure they were genuine adding some were concerned about the level of staffing in their hospital while others wanted to blow the whistle on shortcomings or failures that were raised with hospital officials and ignored.
The revelation follows the CQC warning on Thursday that it is concerned about quality or safety of care in one in four NHS trusts.
CQC also said on Thursday that the failures in the 44 NHS trusts could pose lives at risk.
This comes as an investigation by NHS medical director Bruce Keogh earlier this year had only found 14 trusts with high death rates and care failures.
