The voters started to cast their ballots shortly after 7:00 a.m. on Saturday (22:00 GMT Friday).
A Nobel laureate, incumbent Jose Ramos-Horta of the governing FRETELIN party, is competing against 11 other presidential hopefuls.
This is a second presidential election since the country gained independence from Indonesia. In 1999, following a UN-supervised referendum on the question of self-determination in which the East Timorese voted overwhelmingly for independence, Indonesia surrendered the territory and East Timor became an independent nation on May 20, 2002.
In the early morning, turnout was low but it was likely to get higher later in the day.
At a school building in the village of Balibar near the capital Dili, election workers in yellow T-shirts waited for voters, including Prime Minister Xanana Gusmao, who is scheduled to vote there.
